Emergency NoticeThese Services are not designed for emergency use. Arbitration & Class Action WaiverThis agreement includes a binding arbitration provision and class action waiver detailed in Section 18. By using the Services, you agree to resolve disputes individually through arbitration rather than through court proceedings or class actions. Please review Section 18 carefully before using the Services.

1. About These Terms

Revana Health LLC (“Revana,” “we,” “us,” or “our”) operates a clinical infrastructure and compliance platform that connects patients with licensed, independent healthcare providers. These Terms of Service (“Terms”) govern your access to and use of our website, platform, and all associated services (collectively, the “Services”). By accessing or using the Services, you confirm that you have read, understood, and agree to be bound by these Terms. If you do not agree, do not use the Services.

These Terms incorporate by reference our Privacy Policy, our Notice of Privacy Practices (where applicable), and any supplemental terms presented to you in connection with specific Services.

2. What the Services Include

Revana Health LLC operates as a clinical infrastructure and management services organization (MSO). We do not employ physicians, practice medicine, or provide clinical care. All medical decisions are made by independent, licensed healthcare providers operating through affiliated professional entities in the states where they are licensed to practice.

Through the Services, patients may access: coordination with licensed, independent healthcare providers for telehealth consultations across service lines including hormone replacement therapy (HRT), GLP-1/weight management, peptides, and related wellness and longevity services; facilitation of Good Faith Examinations (GFEs) and clinical clearances required for treatment; coordination with licensed pharmacies and compounding facilities for fulfillment of prescriptions issued by your provider; secure electronic health record management and clinical documentation support, provided through third-party technology platforms; scheduling, billing administration, and care coordination services; and patient education resources and health information materials.

Nothing available through the Services constitutes a diagnosis, treatment recommendation, or medical opinion from Revana Health LLC. All clinical determinations rest solely with your treating provider.

No Insurance Accepted

The Services operate on a direct-pay, cash-pay basis. We do not accept commercial health insurance, are not in-network with any insurance plan, and are not enrolled as providers under Medicare, Medicaid, or any federal or state healthcare program. By using the Services, you acknowledge that you are solely responsible for all fees.

Controlled Substances

Certain controlled substances may be available where permitted by applicable federal and state law. Affiliated providers may prescribe Schedule IV controlled substances, including phentermine for weight management, and Schedule III controlled substances, including testosterone for the treatment of clinically diagnosed hypogonadism or hormone deficiency, in states where the prescribing provider is licensed and where telehealth prescribing is authorized. Schedule II controlled substances, opioids, and benzodiazepines are not available through the Services under any circumstances. Clinical suitability is determined solely by your treating provider.

Pharmacy Services

If your provider issues a prescription, we will coordinate fulfillment with licensed pharmacies or compounding facilities within our network. Revana Health LLC does not dispense medications; all medications are dispensed by licensed third-party pharmacies. You may also request that your prescription be sent to a pharmacy of your choice.

No Provider-Patient Relationship With Revana

Your use of this website or its content does not create a provider-patient relationship between you and Revana Health LLC. A provider-patient relationship is formed only if and when an independent, licensed provider affiliated with the platform accepts you as a patient following an appropriate clinical evaluation.

Product Images, Descriptions & ResultsAny images, photographs, renderings, or depictions of medications, vials, syringes, packaging, or products shown anywhere on the Services — including on our website, marketing pages, and product pages — are stock or illustrative representations for general informational purposes only. They are NOT photographs of the actual product you will receive and do not accurately depict the medication, compounded preparation, vial, container, packaging, labeling, dosage form, fill volume, color, or physical appearance of what is dispensed to you. The actual medication you receive is prepared and dispensed by an independent, licensed third-party pharmacy, and its appearance and packaging will differ from any image shown. Product descriptions, dosages, timelines, and depictions of outcomes are illustrative and educational only; they are not a promise, guarantee, or representation of any specific product, appearance, or result. Individual results vary and no particular outcome is guaranteed.

Compounded and Non-FDA-Approved Medications

Certain medications, including compounded semaglutide, tirzepatide, and other compounded preparations, are prepared by licensed compounding pharmacies pursuant to a valid prescription. Compounded medications are not FDA-approved and have not been evaluated by the FDA for safety, effectiveness, or quality. Your provider will discuss the risks and benefits of any compounded medication with you.

3. Who Can Use the Services

The Services are available to adults 18 years of age or older physically located within a state where our affiliated providers are licensed to practice. Some Services may only be available to individuals 21 or older. Minors may not use the Services under any circumstances. Eligibility does not guarantee that you will receive any particular treatment — clinical suitability is determined solely by your provider.

4. Creating and Managing Your Account

You agree to provide accurate, current, and complete information during registration and to keep your account information up to date. You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. Notify us immediately at support@myrevana.com if you suspect unauthorized access.

5. Your Privacy and Health Information

Our Privacy Policy explains how we collect, use, store, and share your information. Our Notice of Privacy Practices describes how protected health information may be used and disclosed under HIPAA and applicable law. Both are incorporated into these Terms by reference. Washington residents should also review our My Health My Data Privacy Policy.

6. Platform Ownership and Permitted Use

All content, features, software, and functionality of the Services are owned by or licensed to Revana Health LLC. We grant you a limited, personal, non-exclusive, non-transferable, revocable license to access and use the Services solely for your own personal, non-commercial healthcare purposes. All other rights are reserved.

7. Your Content

When you submit information through the Services, you grant Revana Health LLC, our affiliated provider entities, and our service providers a license to use, store, process, and share that information as necessary to deliver the Services, coordinate your care, maintain records, and operate our platform. Information that constitutes PHI is governed by our Notice of Privacy Practices and applicable law.

8. Copyright Infringement Claims

We comply with the Digital Millennium Copyright Act (17 U.S.C. § 512). To submit a copyright infringement notice, contact our Designated DMCA Agent at: 3230 Southgate Circle, Suite 77, Sarasota, Florida 34239 (Attn: DMCA Agent). Your notice must include the statutory elements required under the DMCA.

9. Prohibited Conduct

You agree not to use the Services in any manner that violates applicable law; impersonates any person or entity; introduces malware; attempts unauthorized access; involves scraping or automated data collection without consent; involves commercial exploitation, resale, or redistribution of the Services; transmits spam; disrupts the Services; harasses providers, staff, or users; removes proprietary notices; creates competing products using our content; or enables any third party to do the above.

10. No Warranties

THE SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND. To the fullest extent permitted by law, Revana Health LLC and its affiliated entities disclaim all warranties, whether express, implied, statutory, or otherwise, including any warranties of merchantability, fitness for a particular purpose, title, accuracy, non-infringement, availability, security, or freedom from errors. Nothing in the Services constitutes medical advice, and Revana makes no representation that any treatment, medication, or clinical approach is safe, appropriate, or effective for you.

11. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, Revana Health LLC and its affiliated entities shall not be liable for any indirect, incidental, special, consequential, punitive, or exemplary damages arising out of or relating to your use of or inability to use the Services. In no event shall Revana’s total cumulative liability exceed the greater of one hundred dollars ($100) or the total amount you paid for the Services during the twelve (12) months preceding the claim. California residents waive California Civil Code § 1542.

12. Your Indemnification Obligations

You agree to defend, indemnify, and hold harmless Revana Health LLC, its affiliated professional entities, and their respective officers, directors, employees, agents, contractors, licensors, and successors from any claims, liabilities, losses, damages, costs, and expenses (including reasonable attorneys’ fees) arising out of your use of the Services in violation of these Terms; your submission of false information; your violation of any law; or your infringement of any third-party rights.

13. Third-Party Services

The Services may include links to or integrations with third-party websites, tools, platforms, or services not operated by Revana Health LLC, including EHR and telehealth vendors, laboratories, and payment processors. We are not responsible for their content, privacy practices, security, or availability. Your use of third-party resources is subject to their terms and at your own risk except as required by law.

14. Changes to These Terms or the Services

We may update these Terms at any time. Material changes will be communicated by updating the “Last Updated” date and, where required by law, by direct notice. Your continued use after any update constitutes acceptance of the revised Terms. We may modify, suspend, or discontinue the Services at any time without notice.

15. Fees, Payment, and Subscriptions

Fees are presented at checkout before you confirm any purchase. By submitting payment, you authorize Revana Health LLC, our affiliated entities, and our payment processors to charge your designated payment method for all applicable fees. Prices are subject to change.

Subscriptions and Automatic Renewal

Certain care plans, treatment programs, and memberships are offered as recurring subscriptions with automatic renewal. Before you enroll, we will clearly disclose the subscription’s recurring nature; the billing frequency; the amount charged each cycle; and the deadline and method to cancel. Your subscription automatically renews until you cancel. You may cancel at any time through your account settings or by contacting support@myrevana.com. Cancellation takes effect at the end of your current billing cycle.

Non-Refundable Charges

Due to the regulated and perishable nature of prescription medications, all charges are generally non-refundable once an order has been processed. Exceptions may apply for product damage, loss in transit, or treatment unavailability as described in Section 16.

16. Refunds and Treatment Availability

Treatment plans are subject to product availability, applicable regulations, and the clinical judgment of your provider. If a prescribed treatment becomes unavailable due to supply shortages, regulatory changes, or circumstances outside our control, you may be eligible for a prorated refund based on the unshipped portion of any prepaid plan; approved refunds are processed within 30 days. If your prescribed treatment is unavailable, your provider may recommend a clinically appropriate alternative, which you may accept, or you may request a refund or platform credit. Dissatisfaction with clinical results, a change of mind, or a decision to discontinue treatment is not, by itself, grounds for a refund.

17. Ending Your Use of the Services

You may stop using the Services at any time. Revana Health LLC may suspend or terminate your access at any time, with or without notice, for any reason including violation of these Terms. Sections 5, 7, 10, 11, 12, 18, and 20 survive termination. Following termination, your medical records will be retained by the applicable provider entity in accordance with applicable law and remain accessible to you as required by law.

18. Governing Law and Dispute Resolution

ImportantTHIS SECTION REQUIRES YOU TO RESOLVE MOST DISPUTES WITH REVANA HEALTH LLC THROUGH BINDING INDIVIDUAL ARBITRATION. BY AGREEING TO THESE TERMS, YOU WAIVE YOUR RIGHT TO A JURY TRIAL AND YOUR RIGHT TO PARTICIPATE IN ANY CLASS ACTION PROCEEDING.

These Terms are governed by the laws of the State of Florida, without regard to conflict of law principles. Non-arbitrable disputes shall be resolved exclusively in the state or federal courts located in Sarasota County, Florida. Except as set forth below, all disputes will be resolved through binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules. Before initiating arbitration, the party asserting a claim must send written notice and make a good-faith effort to resolve the dispute within 30 days.

Exceptions: Either party may bring an individual claim in small claims court; either party may seek injunctive or equitable relief in court to protect intellectual property or prevent irreparable harm; and nothing prevents you from filing a complaint with a government agency.

Class Action Waiver: You and Revana Health LLC each agree that any proceeding will be conducted only on an individual basis. You waive any right to bring or participate in a class, collective, consolidated, or representative action.

19. Communicating With Us

By providing your phone number and opting in to receive text messages, you consent to receive recurring SMS messages regarding your account, treatment coordination, order updates, and — where specifically opted in — promotional communications. Standard message and data rates may apply. Reply STOP to opt out. Consent to receive communications is not a condition of using the Services.

20. Electronic Communications and Signatures

By using the Services, you consent to transact business with us electronically and to receive communications, agreements, disclosures, notices, and records in electronic form. You agree that your electronic acknowledgments, acceptances, and signatures have the same legal effect as handwritten signatures, consistent with the federal E-SIGN Act and applicable state law.

21. Accessibility

Revana Health LLC is committed to making the Services accessible to individuals with disabilities. If you have difficulty accessing any part of the Services or need a reasonable accommodation, contact support@myrevana.com and we will work to provide the information, service, or accommodation you need through an alternative means.

22. General

These Terms, together with the documents incorporated by reference, constitute the entire agreement between you and Revana Health LLC regarding the Services and supersede all prior agreements. If any provision is held unenforceable, the remaining provisions remain in full effect. Our failure to enforce any provision is not a waiver. You may not assign these Terms. We may freely assign or transfer these Terms in connection with a merger, acquisition, financing, reorganization, or sale of assets. Headings are for convenience only.
