Revana Health, LLC (“we,” “us,” or “our”) provides clinical infrastructure and care coordination services connecting patients with licensed, independent healthcare providers. This Privacy Policy explains what information we collect, how we use and share it, the choices you have, and the rights you hold under applicable law. It is incorporated into our Terms of Service. Separately, our Notice of Privacy Practices governs protected health information (PHI) subject to HIPAA and applicable state health privacy laws. What Information We Collect and How
Information You Give Us Directly
When you register, complete intake forms, request a consultation, or otherwise interact with the Services, you may provide: contact and identity information (name, date of birth, address, phone, email); health and clinical information (medical history, symptoms, medications, lab results, prescriptions, photos submitted for clinical review); payment and billing information collected by our payment processors; identity verification documents where required; communications you send us; and any other information you choose to provide.
Sensitive Personal Information
Some information may qualify as “sensitive personal information” under applicable state law, including health and medical information, precise geolocation, contents of communications, and financial account details. We collect and use sensitive personal information only as necessary to provide the Services or as permitted by law, and do not use it to infer characteristics about you beyond what is necessary to provide the Services.
Information We Collect Automatically
Our systems and third-party tools automatically collect device and browser information (IP address, browser type, operating system, device identifiers); usage data (pages visited, time on pages, links clicked, search terms, session duration); general location derived from IP or, with permission, precise GPS; and cookie and tracking data as described in our Cookie Notice.
Information We Receive From Third Parties
We may receive information about you from licensed healthcare providers, affiliated professional entities, pharmacies, and laboratory services involved in your care, and technical and usage information from analytics providers and advertising platforms.
How We Use Your Information
We use the information we collect to facilitate access to licensed providers and coordinate consultations, Good Faith Examinations, and treatment clearances; process prescriptions and coordinate fulfillment; manage your account, verify identity, process payments, and maintain records; communicate about your care, account, orders, and platform updates; provide customer support; improve the platform through internal research and analytics; comply with applicable laws; detect and prevent fraud and unauthorized use; pursue or defend legal claims; and for any additional purpose disclosed at collection or with your consent. Where we handle PHI subject to HIPAA, we use and disclose it only as permitted by our Notice of Privacy Practices.
How We Share Your Information
We share information only as necessary to deliver the Services, as permitted by law, or with your consent, and store and process it in the United States: with licensed healthcare providers and pharmacies involved in your care; with service providers and vendors (payment processors, cloud storage, analytics, support, security) contractually required to use your information only as directed; with affiliated professional entities under our PC-MSO structure; for legal and regulatory disclosures; in connection with business transactions such as a merger or sale of assets (with notice before your information becomes subject to a materially different policy); with advertising networks using de-identified or hashed information only (never PHI or consumer health data); and with your consent.
Online Analytics and Advertising
We use analytics tools such as Google Analytics to understand how the Services are used. To opt out of Google Analytics, install the Google Analytics Opt-out Browser Add-on at https://tools.google.com/dlpage/gaoptout. We may work with third-party advertising partners to serve targeted advertisements; update your preferences through our Cookie Consent tool or browser settings. If you opt in to SMS, you consent to recurring marketing and service messages; reply STOP to opt out. Consent to marketing is never a condition of using the Services.
Your Choices and Rights
Depending on where you reside, you may have rights to: know/access the personal information we hold; correct inaccurate information; delete your personal information (subject to legal and operational exceptions); restrict processing; obtain your information in a portable format; opt out of sale or sharing for cross-context behavioral advertising; limit use of sensitive personal information; non-discrimination for exercising your rights; and withdraw consent. To exercise any right, email support@myrevana.com with your full name, state of residence, and email address. If your request is denied, you may appeal by emailing support@myrevana.com with subject line “Privacy Rights Appeal,” and you may file a complaint with your state’s attorney general.
Cookies and Tracking Technologies
We use the following categories of cookies: strictly necessary (required for the Services to function); functional (saved preferences, language, session continuity); analytics (aggregated usage understanding); and marketing/advertising (relevant advertising and campaign measurement). Manage preferences through our Cookie Consent tool or your browser settings. We do not currently respond to browser “Do Not Track” signals because there is no consistent industry standard.
How We Protect Your Information
We implement administrative, technical, and physical safeguards including access controls, encryption in transit and at rest, security monitoring, and regular risk assessments. No security measure is perfect — if you believe your account has been compromised, contact support@myrevana.com immediately.
Children’s Privacy
The Services are not directed to individuals under 18. We do not knowingly collect personal information from minors. If we become aware of such collection, we will delete it promptly. If you believe a minor has provided us information, contact support@myrevana.com.
Data Retention
We retain your information as long as necessary to fulfill the purposes described here, comply with legal obligations (including medical record retention requirements), resolve disputes, and enforce agreements. Medical records are retained per state-specific laws, typically a minimum of 7–10 years depending on jurisdiction.
State-Specific Privacy Disclosures
California (CCPA/CPRA)
California residents have the right to know, delete, correct, opt out of sale or sharing, limit use of sensitive personal information, non-discrimination, and to designate an authorized agent. We respond to verifiable requests within 45 days. California’s “Shine the Light” law (Civil Code § 1798.83) permits requests about personal information shared with third parties for direct marketing.
Colorado, Connecticut, Virginia, and Texas
Residents of Colorado (CPA), Connecticut (CTDPA), Virginia (VCDPA), and Texas (TDPSA) have rights to access, correct, delete, and port personal data, and to opt out of processing for targeted advertising, sale of personal data, and profiling. Appeals may be submitted to support@myrevana.com with subject line “State Privacy Appeal.”
Nevada
Nevada residents may request that we not sell their personal information as defined under NRS Chapter 603A by contacting support@myrevana.com.
Washington State — Consumer Health Data
Washington residents have additional rights regarding consumer health data under the My Health My Data Act, including the right to access, delete, and withdraw consent for collection and sharing of consumer health data. Please see our My Health My Data Privacy Policy.
Your Privacy Choices
Do Not Sell or Share My Personal Information
To opt out: email support@myrevana.com with the subject line “Do Not Sell or Share My Personal Information” and include your full name and email; use the Cookie Consent tool to disable non-essential tracking; or submit a request through myrevana.com/privacy-choices if a web form is available. Once you submit your opt-out, we will stop selling or sharing your personal information within 15 business days.
Limit the Use of My Sensitive Personal Information
Under California law (CPRA), you may direct us to limit the use and disclosure of your sensitive personal information to what is necessary to provide the Services. To request this, email support@myrevana.com with the subject line “Limit Sensitive Personal Information.”
Communications Preferences
To opt out of marketing emails, click “Unsubscribe” in any marketing email. To opt out of SMS marketing, reply STOP. You will continue to receive transactional and account-related communications. Certain communications related to your health and safety cannot be disabled while your account is active.
Policy Updates
We may update this Policy from time to time. When we do, we will update the “Last Updated” date. Material changes will be communicated by email or a prominent notice on the Services. Your continued use after any update constitutes acceptance of the revised Policy.
