Revana does not provide medical care, practice medicine, diagnose conditions, prescribe medications, or control clinical decision-making. Medical services are provided solely by licensed healthcare providers affiliated with Arora Health, LLC.
2. Information We Collect
Personal Information – name, address, phone number, email address, date of birth, identification information, and billing details.
Health Information – medical history, symptoms, medications, treatment plans, lab results, pharmacy information, and provider communications which may constitute Protected Health Information (PHI).

Information From Third Parties – providers, medical groups, laboratories, pharmacies, identity verification services, payment processors, and other service vendors.
